Schedule a Speaker
If you would like someone from the CPSO to speak at your organization or event about our initiatives, policies and/or other medical regulatory topics, please complete and submit this Speaker Request Form.
VISA Applications, Embassy Letters and Other Travel Document Certification
We certify your physician's registration with the CPSO on foreign embassy medical forms if your travel requires it.
Note: The standard processing time for all embassy documents is approximately 3-5 business days. We do not offer same-day or expedited document certification services.
We are unable to provide real-time updates on the status of documents undergoing verification. To avoid delays, please ensure you submit the documents as per the instructions below. There is no fee for this service.
To obtain certification:
- Submit completed, original forms, including the physician's original signature.
- Ensure the form clearly shows the physician's full name, signature and CPSO number. Lab results alone are not sufficient.
- Attach an acceptable translation if the form is in a language other than English or French.
- Purchase and enclose a self-addressed “Xpresspost” Canada Post pre-paid envelope to expedite return delivery.
- Mail or courier the forms and pre-paid envelope to the following address:

You can track your parcel’s delivery status to the College and its return after processing. To track your document:
- Keep a copy of your courier company’s name and tracking number(s), and check the parcel’s delivery status via the applicable delivery service’s website.
- If you are sending your parcel to the College via regular mail, please include your email address so we may send you an acknowledgement of receipt.
Once received, we process the request in approximately three to five business days. We will send the certified form back to you using the enclosed, self-addressed Xpresspost envelope. Please note: mail service interruptions may affect delivery and return times.

Request College Data
CPSO updated its data sharing policies to streamline requests. The College will consider requests for data from a standardized data set of active (registered) physicians by requestors seeking the information to assist in continuity of care, health resource planning and management. This means that research and commercial requests will not be eligible.

Underground paid parking is available at College Park, 777 Bay Street. Enter on the east side of Bay Street, south of College.
